Abstract: 

PERCUTANEOUS transluninal coronary ANGIOPLASTY (P.T.CA) is one of the efective treatment of coronary artery disease.This method was described in 1979 and since then has improved rapidly. In this study the results, and the complications of 162 cases of PTCA is described. The procedure was done between the years of 1373 to 1375 in Chamran Hospital in Isfahan. The patients were followed up untill the end of 1380. Primary successful was 80%. The most but rare complications were acute closure disection and also hematoma at inquinal area.

Abstract: 

A number of patients with severe obstruction due to hypertrophic cardiomyopathy have derived benefit at least over the short-term from inventional infarction of a portion of the interventricular septum by the infusion of alcohol into a selectively catheterized septal artery, with reduction of the outflow gradient and improvement in symptoms. This paper contains successful TASH on a symptomatic patient with high LVOT gradient and methods and complications.

Abstract: 

History & Objective: PERCUTANEOUS coronary ANGIOPLASTY (PTCA) had found a widespread acceptance in treatment of coronary artery disease (CAD). Regarding a controversy being raised about early complication of this method in literature, the present study was aimed to study and evaluate the result of application of this method immediately and one year after operation.Materials & methods: The subjects included 46 patients between 40 and 60 years of age (32 male and 14 female). 32 of cases had single vessel, 7 cases had two vessel and 4 cases had three vessel disease. 20 patients underwent stent implant (i.e. 4 cases of primary stenting and 16 cases of Bailout stenting).tread mill test was done for all patients before and 4 months after PTCA.Results: Pre- and post PTCA tread mill test was positive in 46 and 18 cases, respectively. No re-stenosis, severe bleeding, myocardial infarction, or mortality were reported. Discussion: Since the present study suffers from small number of cases, further studies are required to give a better understanding of the issue.

Abstract: 

Background: Practice of interventional cardiology procedures such as Coronary angiography (CA) and PERCUTANEOUS TRANSLUMINAL coronary ANGIOPLASTY (PTCA) has largely increased in recent years and is now changing to a matter of routine in many hospitals. The purpose of this study was to measure skin and organ doses in patients undergoing CA and PTCA and to find a probable relation with cardiologist's work experience.Materials and Methods: A group of 57 patients who were admitted to Nour and Chamran hospitals, Isfahan, Iran, either for CA (n=37) or PTCA (n=20) was checked for skin, eyes and thyroid gland radiation dose. Skin dose measurement was carried out with thermoluminescent dosimeters (TLD-100) placed at three different positions on the patients' bodies. Also the thyroid and eye doses were measured using the same procedure.Results: Mean median, standard deviation and maximum doses for these results are presented. Maximum values found for skin in CA, PTCA, thyroid gland and eyes were 41.00, 73.90, 3.10 and 1.43 cGy, respectively. Average exposure time for CA and PTCA was 4.2±2.6 and 10.8±8.2 minutes, respectively. Radiation dose to the critical areas in each procedure in addition to its relation to the type of procedure was studied.Correlation between maximum skin dose and cardiologists' work experience was also investigated.Conclusion: 85% of the cases in CA and 78% of cases in PTCA received maximum skin dose of lower than 25cGy well below the threshold of 2 Gy suggestions for transient erythema.

Abstract: 

Background: Renal artery stenosis (RAS) is one of the main causes of secondary hypertension and the most preventable cause of RAS is atherosclerosis. Recently, PERCUTANEOUS TRANSLUMINAL renal artery ANGIOPLASTY (PTRA) has become the treatment of choice for RAS. We assessed the outcome of PTRA in a sample of patients referred to heart clinics in Isfahan University of Medical Sciences.Methods: Thirty two patients with RAS and persistent hypertension were selected and underwent of PERCUTANEOUS TRANSLUMINAL renal artery ANGIOPLASTY. Then, they were followed for 4 weeks by a cardiologist. The main outcomes were blood pressure, serum creatinine, cardiac functional state and chest pain. The Repeated Measure of ANOVA and Freidman test were used for statistical comparisons via SPSS14 software.Findings: The mean of systolic blood pressure decreased from 199.4 to 142.2 mmHg during 4 weeks follow-up (P<0.0001); the diastolic blood pressure decreased from 114 to 89.2 mmHg too (P<0.0001). The cardiac functional state increased and chest pain decreased significantly during 4 weeks; but the serum creatinine level showed no significant decrease.Conclusion: PTRA is a useful method for treatment of hypertension in patients with RAS. It decreases the Renin-Angiotensin-Aldostron pathway and declines neurohormonl activities. Thus these changes decrease blood pressure level and cardiovascular morbidity and mortality. It improves renal function, too. By this procedure, the need for antihypertensive drugs will decrease and it has economic benefits for patients too.

Abstract: 

Background: PERCUTANEOUS TRANSLUMINAL coronary ANGIOPLASTY (PTCA) with or without stenting is a well established technique for coronary revascularization. The injury to the myocardium during the technique was investigated by measuring serum levels for cardiac troponin I and activity of Creatine Kinase MB Isoenzyme (CK-MB), the sensitive and relatively specific biomarkers of cardiac injury. In this study, to detect minor myocardial injury after PTCA, we evaluated serum levels of CTNI and CK-MB after PTCA.Methods: The serum levels of CTNI and the activity of CK-MB were measured by standard methods in 100 patients undergoing elective PTCA, before and 12-24h after the procedure. Patients who had hepatic, renal and thyroid disease; recent myocardial infarction; electrocardiographic changes, chest pain and myocardial infarction after intervention were excluded from our study.Results: Meaningful differences between the mean levels of CTNI in the samples obtained before and after PTCA; 0.67 ± 0.58 ng/ml VS 1.19 ±0.94 ng/ml (P= 0.000) and those of CK-MB activities; 22.89±8Iu/l VS 26.38±13.39 (P=0.010) were noticed. Stenting place had no effect on the measured parameters. Relationships between history of Hypertension, Diabetes, Hyperlipidemia, Previous myocardial infarction, Smoking and elevations of CTNI and CK-MB levels weren’t statistically significant.The frequency of increased levels of CTNI and CK-MB in women was significantly higher than men. Conclusion: We concluded that minor cardiac injury following PTCA may increase the serum level of CTNI and serum activity of CK-MB. CTNI is more sensitive than CK-MB in detecting of the injury and placing stent has no effect on the release of the parameters during the PTCA.
